My thinking is Leisuresuit Larry Uncut and Uncensored for PC.
www.larryuncut.com
It already has the rating.
I know there were games with nudity(and/or sex?) in them before but i think that was before the rating system.
(CD-i anyone?)
I got the site in a email from Vivendi Universal(?) a week or so ago that was one of the links in it.

Adults only means 18+? Can't the 16 year olds in this world look at cartoon nudity anymore?

The rating system began in 1994. And you can check all the ratings on the esrb site. Lots of pathetic porn games have the adults only rating. But some more mainstream ones too. Singles for example.

I assumed that there were no AO games because i had never heard anything about them.

FYI
ESRB began in 1991 or earlier.
Off the top of my head i have a game from 1992(Mortal Kombat) rated by the ESRB.(Might have some earlier if i look.)
I say 1991 though because i remember hearing somewhere that MK was advertised in 1991 and that was one of the major contrinuting factors to the ESRB's creation.

Upon further inspection you are correct sir.
It is rated

MA-13
Parental Discretion Advised
Mature Audiances

but does not wear the official ESRB tag.
Now i wonder who made up the ratings Pre-ESRB?

Those ratings evolved into the ESRB from what I remember.  I'd have to look that up though.  As for games that had the AO rating, I know I have several shrinkwrapped 3DO games like Blonde Justice that are rated AO.  There are others as well.

The pre- ESRB ratings were up the individual game companies. Sega and Nintendo both had there own, and that's what started in 1991.
