Definitely an older Shin Megami Tensei game, and given that it is in English, I'd say a fan-translated one (wrong aspect ratio for a remake), so....

Shin Megami Tensei for the Super Famicom.

Kyūyaku Megami Tensei, or Kyuuyaku Megami Tensei. Common translation mistake, but GameFAQs has it as Kyukakku.
